Before your first appointment you will need to fill out a Health Information Form, a Covid-19 Waiver and sign an Office Policy Form. Please, plan to arrive 20 minutes before your scheduled appointment time to do that in the office.

You can also fill out the forms ahead of time at home. Just download the forms, print and fill them out and bring them to your appointment.
